Integrated Practice


Sometimes children, young people and their families need extra help and support.

Integrated Practice (IP) is part of the Every Child Matters agenda and aims to give all children the support they need to achieve the five outcomes:

  • Be healthy

  • Stay safe

  • Enjoy and achieve

  • Make a positive contribution

  • Achieve economic well-being

  • Schools, doctors, social workers, police and others in Hertfordshire now work together to make sure children and young people get the right support at the right time.

 

IMPLEMENTATION OF STEP UP/STEP DOWN PROCESS

The Step Up/Step Down process has been strengthened to improve the experience for children and families where their needs escalate and de-escalate, specifically to ensure a smooth transition across the interface between the CAF (Common Assessment Framework) process and specialist/safeguarding services

A series of process diagrams have been created to show different types and levels of intervention and how a Step Up/Step Down process aims to address the ongoing needs of families, taking into account the issues raised. The processes outline work across the partnership of specialist, targeted and universal support (continuum of needs) and aim to enable a deliverable and effective transition for families, ensuring children are protected and families supported by a range of practitioners working together.
